Alzheimer’s Survey

Here is an important statewide initiative through the State of New Hampshire. The NH Subcommittee on Alzheimer’s and Other Related Dementias has launched a Needs Assessment Survey to better understand how we can support individuals living with dementia and the people who care for them.

This survey is open to individuals living with dementia, caregivers, healthcare professionals, and community members who have relevant experience or insight. It takes approximately 5 minutes to complete, and all responses are completely confidential.
